MEDICATION ADMINISTRATION GUIDELINES
For Prescription AND Over-the-Counter (OTC) Medications

  1. Medication Orders Required for ALL medications
    Medication may only be administered with:
    • A written order from the licensed prescriber including: medication name, dose, route, time/frequency, condition being treated, duration, and potential side effects; and
    • Parent/guardian signature at the bottom of the order confirming consent.

Verbal or telephone requests will not be accepted

  1. Medication Delivery
    All medication must be delivered directly to the Health Office by a parent/guardian or designated adult. Medication orders and medication must be submitted together at the time of delivery.

Students may not transport medication to school or on the bus under any circumstances.

  1. Medication Containers
    Medications must be in:
    • The original pharmacy-labeled prescription container; or
    • Original OTC packaging.

Labels must clearly identify the medication name and strength.

  1. Storage
    Medications will be stored securely in the Health Office unless approved by both physician and health office staff for self-carry/self-administration.
  2. Renewal of Orders
    Medication orders must be renewed annually and whenever there is any change in medication, dosage, or administration instructions.
  3. Self-Carry / Self-Administration
    Students may self-carry medication only with:
    • Written prescriber authorization,
    • Written parent/guardian consent and
    • Approval by the school nurse.

The student must demonstrate knowledge of the medication, purpose, dosage, frequency, and side effects. The school nurse retains final approval and may revoke self-carry privileges if safety concerns arise.

  1. Field Trips / School Activities
    Approved self-carry medications may accompany students during field trips and school-sponsored activities in accordance with district policy.
  2. Anaphylaxis / Emergency Medication
    Students with physician-documented anaphylactic allergies must provide prescribed emergency medication (e.g., epinephrine) and required orders. If emergency medication is not required, physician documentation must be provided.
  3. End of Year Medication Pick Up

All student medications (both prescription and OTC) must be picked up by a parent/guardian or authorized adult NO LATER THAN the last day of student attendance/classes for the school year. Medications not picked up by this date will be disposed of in accordance NYS medication disposal guidelines. Medications WILL NOT be sent home with students.
