PHYSICAL EXAMINATIONS
The NYS Education Department also requires proof of physical exam for all new students, special education students in his/her triennial year, new entrants, as well as students who are entering grades Pre-K, K, 1, 3, 5, 7, 9 and 11. The physical exam can be completed one year prior to the start of the current school year.
For optimal continuity of health care, it is recommended that the family's private health care provider complete medical examinations. Parents/guardians must furnish a health certificate signed by a duly licensed physician, nurse practitioner or physician's assistant to the health office by September 20th, for mandated examinations. Any student, who has not supplied the health office with a mandated physical by that date, will be scheduled to see the school physician.
